Documents Requirement

1. Passport or travel document with validity not less than 6 months with Two Blank Pages, and is not damaged.

2. Visa Application Form: Visa Application Form :Duly filled & Signed

3. Two recent photographs of the applicant. Please check details under the tab Photo Specifications.

4. Evidence of Transportation

1. Confirmed roundtrip tickets to Thailand
2. Proof of transportation in Thailand (if applicable)

5. In case the applicant is not admitted in hospital: Proof of hotel/accommodation reservation in Thailand is required

1. The reservation must be under applicant’s name, covering total number of days the applicant wishes to stay in Thailand
2. If intend to stay with a Thai resident the applicant must provide;
1. Invitation letter from the host to the applicant
2. Copy of host’s I.D./passport photocopy
3. In case the host is not Thai, the applicant must provide the host’s working records in Thailand, e.g. work permit.

6. From India:

1. Original Reference letter from Indian Doctor/ Hospital.
2. Copy of all medical reports

7. From Thailand

1. Original letter of acceptance from Thai Hospital

8. Evidence of adequate finance (Any of below)

1. Original receipt of endorsement of 700 USD/person or
2. Original Bank Statement from last month with minimum balance equally as 700 USD/person with bank stamp and signature or
3. Copy of credit Card issued by a bank or financial institution along credit card statement from last month.

Photo Specifications

Colour photographs

Not older than 3 months

Size 3.5x4.5 cm

White background

Dark coloured clothes

Full face must be visible, with face and eyes in correct position to the camera, and teeth invisible.

No sunglasses, hat or any head covering (unless it is warn due to religious beliefs)

Photographs must be clear, of good quality, and are not digitally modified.

Note: The Royal Thai Consulate-General Kolkata has full right to ask applicant to change the photographs.

Processing Time

Processing Time will be approximately 5 working days.

Note :

If there is any further verification required on the submitted documents there will be a further delay in the above mentioned processing time.

The travel date should be minimum seven working days from the date of submission at Thailand Visa Application Centre.
